摘要 当今社会，慢性非传染性疾病发病率越来越高，健康管理作为慢性非传染性疾病预防控制的重要手段越来越受到广泛关注。从2008年开始，南通市相继出台了关于健康管理的相关文件，各项健康管理防治工作开始一步步落实，目前已初见成效。本文基于南通市慢性非传染性疾病健康管理的现状，从实际应用出发进行研究，探讨南通市健康管理现存问题，主要问题如下：健康管理服务对象局限；健康管理服务内容欠缺；缺乏健康管理的专业人才；健康管理服务模式过于传统。针对以上问题进行了如下的原因分析：对健康管理服务人群认知的局限；健康管理服务内容制定的不科学；健康管理人才培养制度不健全；健康管理服务模式缺乏创新。本文是在结合南通市实际情况的基础上针对发现问题提出如下改善措施：加强对健康管理服务人群的认知；丰富健康管理实施的具体内容；加强健康管理人才队伍建设；创新健康管理服务模式。希望通过本文研究为慢性非传染性疾病的健康管理提供合理可行的建议。

Research on Health management of chronic non-communicable
disease in Nantong
Abstract: In modern society,because of high incidence of chronic non-communicable diseases，Health management as an important means of prevention and control of chronic non-infectious diseases are increasingly widespread concern.Starting from 2008,Nantong unveiled a related paper on health management, Health management control efforts started a step by step implementation,Currently has the effect. This article is based on present situation of health management in Nantong ,Proceed from the practical application of research,Discussion on health in Nantong management existing problem,Now found the problem is as follows:Health management services are limited ;Health management elements lack of monitoring intervention;The lack of health professionals;Health management services model was too traditional.Analysis of the causes of these problems are as follows:Cognition on health management services group limited;Health management services not developed by science;Inadequate health management training systems;Lack of innovation mechanism of health management services model.Nantong city, based on the combination of these reasons and realities made the following conclusions:Enhanced knowledge about health management services group;Rich health management implementation specific details;Strengthening health management personnel construction;Innovation model of health management services.Hope through research for the health management of chronic non-infectious diseases provide a feasibility proposal.
